4Health Requirements
- Immunizations
- Physical Exam
- Birth Certificate
- Health History
- Vision Hearing Screen
5Immunization Requirements
- Public Health Code requires schools to report
students immunizations - Student needs Hepatitis B, Tetanus, MMR,
Varicella (or has had Chickenpox) Polio - Follow up with family doctor or local health
department if your child has immunization needs
6Physical
- St. Johns Public School Policy requires students
to have a current physical when enrolling within
the last year - American Academy of Pediatricians recommends
children have routine Well child Checkups age
4,5, 6
7Birth Certificate
- The School District needs a copy of the original
birth certificate - Required by law
8Health History
- Any pertinent health information medications,
allergies, chronic conditions, emotional problems
or health concerns - Take appointments and then phone conversation
regarding
9Vision Hearing Screening
- Students entering school are required to have
vision hearing exams - Documented on physical done by physician, NP or
PA - Health Department Schedule to sign up tonight
for an appointment
